Enrichment Clubs Enrollment Form 
* If you are a member of one of our programs and would like to use your  FREE  Enrichment class, please RSVP for the club via the BAND App on the event calendar. This form is for families that have used their free credit for the term and would like additional classes or that are solely signing up for our enrichment classes.

* Separate form must be completed for each student

* Listed price is the cost for each class session/meeting
